Candidates must reside within 50 miles of an Elevance Health Pulse Point location.Work schedule: Monday - Friday 8:00am to 5:00pm depending on your zone.A proud member of the Elevance Health family of companies, Carelon Behavioral Health, formerly Beacon Health Options, offers superior clinical mental health and substance use disorder management, a comprehensive employee assistance program, work/life support, specialty programs for autism and depression, and insightful analytics to improve the delivery of care.The Care Coordinator is responsible for coordinating the delivery of care for members.How you will make an impact: Implements, coordinates, monitors, and evaluates options and services to meet member's health needs and ensures appropriate use of clinical resources.Monitors delivery of care across all markets.Maintains daily admissions and discharge records.Expedites and coordinates appointments for assigned hospitalized patients.Authorizes and coordinates services.Keeps all providers involved with member's care updated on appointments, condition, and additional clinical support needed.Requests and gathers necessary medical records.Maintains accurate and complete documentation in Case Management database.Minimum Requirements:Requires a H.S. diploma or equivalent and a minimum of 2 years of experience in Case Management Care Coordination in an HMO environment; or any combination of education and experience which would provide an equivalent background.Preferred Capabilities, Skills and Experiences:Medical Assistant Certification preferred.For candidates working in person or remotely in the below location(s), the hourly* range for this specific position is $17.73 to $29.13.Locations: California; NevadaIn addition to your salary, Elevance Health offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
