Job Summary

We are looking for a Community Care Coordinator to join our team at El Futuro. In this role, you will work closely with community partners to improve access to behavioral health care for Spanish-speaking underserved populations in Wake, Durham, and Orange Counties.

Key Responsibilities
  • Deliver interventions created by Licensed Clinical Therapists to support community partnerships.
  • Provide case management services to maintain a consistent caseload of participants.
  • Collaborate with healthcare staff to ensure an integrated care model.
Requirements
  • A high school diploma or equivalent is required; interest in mental health and integrated care is preferred.
  • Bilingual fluency in spoken and written Spanish and English is essential.
  • Experience working in the specific service area or familiarity with its resources is beneficial.
  • Alignment with El Futuro's mission to nurture stronger families and live out their dreams is crucial.
Salary and Benefits
