We go beyond physical health by addressing other factors that impact patient well-being. Our integrated care teams, which include physicians, nurses, behavioral health specialists, and more, spend up to 50% more time with patients, providing compassionate, personalized care that brings better health outcomes.
We are seeking a Primary Care Physician (PCP) to join our team-based care environment. As a PCP at CenterWell Primary Care, you will work as a lead in our team-based care environment. We are a value-based care provider focused on quality of care for the patients we serve. Your responsibilities will include evaluating and treating center patients in accordance with standards of care, following level of medical care and quality for patients and monitoring care using available data and chart reviews.
You will also assist in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care. As an active participant and key source of medical expertise with the care team, you will help Regional Medical Director and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor. Additionally, you will complete all medical record documentation in a timely manner working with a quality-based coder to optimize coding specificity.
