Clinical Care Coordinator

**Salary:** $60,000 - $85,000 per year (dependent on experience)
About the Role:
The Clinical Care Coordinator will work closely with our clinical teams to coordinate patient care, manage communications, and ensure seamless transitions between departments. This role requires excellent organizational skills, strong attention to detail, and the ability to maintain confidentiality and handle sensitive information.
Responsibilities:
* Coordinate patient care and manage communications with clinical teams
* Ensure accurate and timely documentation of patient information
* Assist with scheduling appointments and managing patient flow
* Participate in educational activities, departmental committees, and research projects as assigned
Requirements:
* High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred
* Minimum 1 year of experience in a healthcare setting, preferably in a coordination or administrative role
