Job Description:

We are seeking an experienced Nurse Care Coordinator to join our dynamic team at Duke HomeCare & Hospice. As a key member of our interdisciplinary team, you will play a vital role in coordinating patient care, ensuring seamless transitions between hospital and home settings.

Key Responsibilities:

  • Develop and implement comprehensive care plans tailored to individual patient needs">
  • Collaborate with healthcare providers to ensure timely and effective communication">
  • Foster strong relationships with patients, families, and caregivers to promote optimal outcomes">
  • Coordinate logistics, including transportation and equipment arrangements">
  • Maintain accurate records and reports, adhering to regulatory standards">

Required Skills and Qualifications:

To be successful in this role, you should possess:

  • ">
  • Current RN licensure in North Carolina or compact state">
  • 2+ years of experience in med-surgical, critical care, or home health/hospice/infusion">
  • Strong organizational and communication skills">
  • Ability to work autonomously and prioritize tasks effectively">
  • Knowledge of Medicare, Medicaid, JCAHO, and DFS standards">
