Severe and Disabling Mental Illness (SDMI) Waiver Case Management is a program offered through AWARE for people 18 years and older who require long-term community support due to mental health symptoms. Responsibilities: Coordinate meetings with case managers, clinicians, community-based providers, and/or support workers; Maintains a caseload established with the Service Director in order to work closely with programs in assigned service areas; Assesses and evaluates adults to determine the appropriateness and need for case management services; Assists in the person's entry into service and monitors the needed services throughout service system; Provides coordination and comprehensive development of an Individual Treatment Plan (ITP) and monitors that plan as required by established rules; Follows State of Montana and A.W.A.R.E., Inc. rules, policies, procedures, and guidelines as they apply to this service; Assures that any person serviced by Adult Case Management is screened for and assisted in enrollment in the SDMI program; To learn more please visit AWARE - SDMI .
