Responsibilities
  • Provide skilled nursing and coordinate care to patients and their families in their homes, catering to various living situations and lifestyles.
  • Monitor, assess, evaluate, and plan interventions regarding patients\u2019 physical and mental statuses.
  • Execute continuity of care through thorough communication and information exchange between clinicians and administrative staff, floating between home health and hospice as needed.
  • Create Aide Care Plans for Home Health Aides and oversee care provided, ensuring compliance.
  • Participate in team scheduling and \u201Con-call\u201D rotation, assessing patients\u2019 care needs via telephone and triaging as necessary.
Qualifications
  • Valid state driver\u2019s license
  • Travel and On-Call availability required
  • Automobile liability insurance and registration
  • Dependable transportation in good working condition
  • Ability to drive in all weather conditions
  • Successful background check
