The HHF10 team supports individuals living in various apartment complexes throughout Norfolk. While our office is located in Ghent, this role involves being on the go frequently to provide support in the community. Our dynamic team consists of four dedicated staff members supporting ten individuals, fostering a close-knit and collaborative environment. You will be able to help with banking, developing goals, and plan writing. You will also lead in assisting those you support with managing their money and leading in health care management for those accepting services. The Service Coordinator is a rewarding, fulfilling position for an organized and energetic personThe schedule for this position is: Sunday 9a-5p, Monday 8a-4p, Tuesday 8a-4p, Wednesday 9a-5p and Thursday 9a-5p.Full-time BenefitsHealth insurance includes vision, (up to 93% paid by Hope House)Dental insurance (100% paid by Hope House)Life insurance (100% paid by Hope House)Optional benefits: additional life insurance, disability insurance, and flexible spending accountsGenerous leave including 12 paid holidays401k**Position Description: Direct Support Professional at the Hope House 10 team**As a Direct Support Professional at Hope House Foundation, you will be vital in assisting individuals with disabilities in completing daily living activities. Your responsibilities will include, but are not limited to:- **Daily Living Assistance:** Teach and assist persons with daily routines such as cooking, cleaning, laundry, hygiene, meal planning, grocery shopping, banking, and medication management.- **Transportation:** Use your vehicle, as needed, to facilitate services and community integration.- **Individualized Support:** Provide training based on each individual's Individualized Service Plan (ISP) and assist them in engaging in community activities and leisure interests.- **Rights Advocacy:** Support individuals in exercising their rights and resolving any service-related complaints.- **Behavioral Support:** Implement and lead behavioral support plans, guiding team members as necessary.- **Emergency Response:** Administer first aid when needed and assist in obtaining prompt medical attention for individuals.- **Documentation:** Accurately document services provided while ensuring the confidentiality of all records.- **Crisis Intervention:** Respond to emergency situations and provide crisis intervention as required.- **Professional Representation:** Act as a liaison for persons served, collaborating with family members, agencies, and the community, always positively representing Hope House.- **Team Collaboration:** Attend and actively participate in core team meetings, training sessions, and agency-wide events as defined by the Team Leader. - **Errands and Appointments:** Run errands when needed and assist individuals in scheduling and attending medical appointments, including physical and dental exams.- **Medication Management:** Help with prescription refills and medication administration as prescribed, maintaining accurate Medication Notebooks for each individual.- **Record Keeping:** Keep up-to-date Individualized Service Plan Notebooks and Permanent Records for each person served.- **Financial Management:** Oversee the management of funds belonging to individuals served.- **Team Support:** Maintain communication regarding health-related information and changes among team members, providing support when needed.- **Leadership Duties:** Assume Team Leader responsibilities in their absence.QualificationsAbility to utilize Microsoft Word, Excel, and Quicken programs. Ability to organize and prioritize work tasks.Ability to work effectively in a team.Ability to present self professionally.Ability to self direct work.Ability to communicate effectively, both orally and in writing.Ability to contribute and function within a participative management structure.Ability to carry out the physical requirements of this position, which include but are not limited to lifting up to 50 pounds several times per shift as needed, ascending and descending stairs several times per shift, and sitting, standing, and walking for periods of time throughout a shift as needed.Knowledge of the field of disabilities.Skill in the use of behavioral support practices.Skill to complete satisfactorily: Medication Certification, Medicaid Waiver Certification, First Aid/CPR, Mandt, and other requirements associated withorientation.Skill in decision making, judgment and action in emergency or crisis situations.Skill in maintaining confidential information.Skill in locating and accessing community resources.Must have valid Virginia Driver's License.Must have satisfactory criminal history check.WORKING CONDITIONSWork requires a range of light to moderate physical exertion to assist persons supported with personal care, lifts, and transfers as needed. Work also requires assisting with toileting, changing linens, and other supports for individuals served in completing some activities, and other forms of physical exertion. Work requires the ability to concentrate and pay attention to detail for a majority of work time.Generally good working conditions with potential exposure to communicable diseases with little risk if proper procedures and safeguards are followed.
