Position : Bilingual Patient Care Coordinator - Full time

Who we are looking for:

A friendly, personable, organized and focused individual who is comfortable working with excel spreadsheets, inputting data, making (and answering) phone calls.

Key Responsibilities include:
  • Calling clinics for updates regarding patient information, aspects of therapy, and more.
  • Monitoring the flow of incoming referrals from various referral sources.
  • Communicating with new clients on a daily basis. Following up accordingly.
  • Tracking new clients and referral sources in an excel spreadsheet and sharing the information with the rest of the team.
  • Verifying new client payor benefits monthly and patient financial responsibility, as required and as needed.
  • Staffing new patients according to needed services and disciplines. Working in tandem with the team to achieve staffing goals.
  • Work as a team player with the other employees of Rehab at Home.
  • Perform other duties as assigned by Rehab at Home Management.
Skills We are Looking For:
  • Exceptional Communication Skills
  • Fully Bilingual in English and Spanish
  • Proficient with Microsoft Excel & Word
  • Reading and Writing Proficiency
  • Home Healthcare Software Experience Preferred.
