Neuroscience Patient Care Coordinator

Position Overview:
The Neuroscience Patient Care Coordinator plays a pivotal role in enhancing the quality of care for patients with neurological conditions. This position is integral to the multidisciplinary team, ensuring that patient care is coordinated effectively and efficiently.
Key Responsibilities:
- Collect and analyze data related to patients with neurological diagnoses to support quality improvement initiatives.
- Maintain and oversee data management programs and care protocols, ensuring adherence to best practices within the Neuroscience program.
- Collaborate with healthcare professionals to facilitate comprehensive patient education and resource utilization.
- Participate in the development and implementation of discharge plans, ensuring smooth transitions for patients.
- Systematically gather information to identify trends and opportunities for performance enhancement.
- Assist in the execution of educational programs aimed at patients and the community regarding neuroscience health.
Qualifications:
- Valid RN license in Texas or a compact state.
- Preferred: Bachelor of Science in Nursing (BSN) and a minimum of three years of clinical nursing experience.
- Experience in emergency care is essential.
- Familiarity with quality improvement processes and The Joint Commission standards is preferred.
