North Suffolk's Community Behavioral Health Center (CBHC) program is looking for a Care Navigator to join our team The CBHC Care Navigator coordinates case navigation, participation, and transition for all CBHC programming. They educate and support individuals through the entire process. The Care Navigator also serves as a community liaison, acting as a dynamic team member who supports day to day operations of the programs.

This position is an on-site/in-person role with evenings and/or weekends required.

What you'll do:

  • Coordinate and perform referrals in a prompt, comprehensive and responsive manner which can include:
    • Communication about participants' program adherence to referral sources such as the courts, Department of Children and Families, etc.
    • With collateral providers and community partners such as health care providers, recovery coaches, etc.
    • Participate in outreach and engagement activities with program participants, including follow-up appointments.
  • Run client insurance in real time.
  • Be able to demonstrate competency with eHana and EPIC (training is provided).
    • Reconcile cases from call log with state programs.
  • Support in triaging non-clinical calls.
  • Complete bed searches for CCS, CBAT, detox and other acute levels of care as needed.
  • Under management supervision, identify risk and behavioral crisis needs and bring to the attention of clinical staff as needed.
  • Meet individually with program participants to complete paperwork and finalize other administrative duties.
  • Complete clinical documentation as required in the electronic health record and/or other databases.
  • Maintain databases ands spreadsheets for information tracking, communication, and client assignments as directed.
  • In collaboration with clinical staff, develop and implement individual action plans, monitor and report progress and document as appropriate to credential level.
  • Participate in staff meetings and perform other duties and tasks as assigned.
What you'll need:
  • Education/Skills:
    • Bachelor's Degree in Social Work, Counseling Psychology, Psychology, Expressive Therapies, or related field of study.
    • A minimum of 2 years post-degree experience within a multidisciplinary treatment setting.
    • Group facilitation skills and experience with individuals with substance use disorders required.
    • Length of experience contingent upon educational credentials.
