POSITION SUMMARY:

The Patient Navigator III assists patients, families, and caregivers to navigate the many systems encountered during their healthcare journey. Provides administrative support by performing general clerical work to support patients, providers, and care team members including referral processing, phone triage, DME support, and other duties as needed.

The primary function of this position within the Geriatrics Section is to support the Geriatrics Offsite programs. The Patient Navigator III will also update, cancel and reschedule primary care and home phlebotomy appointments including registration, insurance/coverage verification, appointment scheduling, and other duties to coordinate care for patients.

The Patient Navigator III is a member of our Geriatrics Primary care team. The Patient Navigator primarily supports our Geriatrics Offsite programs which include the Home Care (home-based primary care) and Nursing Home programs.

The Geriatrics Home Care program supports over 500 homebound older adults in the Boston area by bringing primary care services to their home. Our goal is to support patients by helping them to remain at home and age in place.

The Geriatrics Nursing Home program provides both long term and short term care for older adults in six Skilled Nursing Facilities across Boston. Our goal is to help patients have Geriatric focused care during their stay at a skilled nursing facility.

We are looking for a compassionate, patient focused, and holistic person to add to our team.

JOB REQUIREMENTS

EDUCATION:

* Associates' Degree plus three years of related experience or HS Diploma/GED plus five years of related experience.

EXPERIENCE:

* Individual must have experience (paid or volunteer) working with families of various backgrounds. Must have some familiarity navigating community resources and systems.
* Experience in similar or related field (such as community health work or elder services) is preferred.

KNOWLEDGE AND SKILLS:

* Ability to work in a multi-cultural, diverse, decentralized environment, as a member of a health care team.
* Demonstrated customer service skills, including the ability to use appropriate judgment, independent thinking and creativity when resolving customer issues.
* Excellent English communication skills (oral and written) and interpersonal skills are required to interact with internal and external contacts in a courteous and patient focused manner.
* Ability to handle multiple tasks and responsibilities at the same time effectively
* Knowledge of software applications such as Microsoft Word and Excel.
* Ability to interpret and record data and understand basic medical terminology.
* Must be able to maintain strict confidentiality of all personal/health sensitive information.
