Job Summary

We are seeking a highly skilled and compassionate Dementia Care Navigator to join our team at Tufts Medicine. As a key member of our Clinical Care and Operations Team, you will play a vital role in supporting patients and families affected by dementia, ensuring seamless transitions across the care continuum.

Key Responsibilities
  • Provide telephonic patient outreach to ensure transitions of care, including follow-up appointments, prescriptions, and homecare services.
  • Act as a liaison between providers and clinical resources to ensure patients are actively involved in decision-making for all aspects of care.
  • Coordinate with hospital staff, including continuity of care department, to ensure shared patients receive comprehensive care.
  • Monitor emergency department utilization and communicate information to assigned clinical staff for real-time intervention.
  • Develop trusting relationships with patients and families, engaging respectfully and with utmost attention to service.
Requirements
  • High school diploma or GED.
  • Two (2) years of experience in a clinical setting (nursing unit, care management, physician's office).
  • Preferred qualifications: College Degree, Medical Assistant (MA) certification, and three (3) years of related experience.
