About the Role

We are seeking a highly skilled Care Coordinator to join our team at NuvoAir. As a Care Coordinator, you will play a critical role in ensuring that patients with cardiopulmonary diseases receive the best possible care and support.

Key Responsibilities
  • Proactively identify and work with patients to provide coordination and navigation of care and support across health and care services.
  • Develop personalized care plans to address patients' physical, mental, and preventative health needs.
  • Manage and support patients' healthcare needs via phone conversations and in-person visits.
  • Connect patients with resources such as nurses, respiratory therapists, and doctors who can assist them in meeting their care needs.
Requirements
  • Healthcare experience as a background in the medical field is a must have.
  • Must have experience working with patients with pulmonary diseases.
  • Exhibits a startup-ready mindset, adaptable to a dynamic environment where patient needs are prioritized.
  • Passionate about health and wellness, often engaging in conversations about healthy habits.
