General Summary:

Under the direct supervision of the Program Director, the Care Coordinator will provide recovery planning support to Adolescent Clubhouse members and case management services to the families.

Principal Responsibilities and Duties:

  1. Screen and introduce new members and their families to the Clubhouse.
  2. Assist each member and his/her family in developing their personal goals and family participation plan. The plan will assist in guiding involvement in Clubhouse activities and include personal goals and targets that the participants themselves wish to achieve in their recovery from substance abuse addiction.
  3. Assists clients to assess their situation and provide basic case management services including accessing entitlements and other social services and provide referrals to appropriate and recommended community services.
  4. Makes follow-through contacts with clients and other agencies to ensure that needed services have been provided
  5. Consults with county/community/agencies to identify and develop resources for client assistance.
  6. Intervene in crisis situations as needed.
  7. Support Senior Peer Leadership Specialist with activities when necessary.
  8. Reviews programmatic and personal documents, creates and maintains case contact notes and records, and performs related functions.
  9. May provide transportation to clients as needed.
  10. Performs other duties as assigned.
Knowledge, Skills, and Abilities Required:
  1. Requires two years of experience in casework, guidance or counseling, and peer navigation.
  2. Requires a Bachelor of Arts in a Human Services related field. Masters degree is preferred. Two years of experience can be substituted for two years of education.
  3. Ability to communicate well with parents, community, professionals and deal with a variety of personalities and situations with youth, staff and parents.
  4. Sensitivity to substance abuse issues among teens and their families.
  5. May provide transportation in a company-owned vehicle. Must possess a valid state driver's license with clean driving record (3 or fewer points).

  1. Generally, work occurs in a programmatic environment where there is no physical discomfort due to temperature, dust or noise. Noise levels may fluctuate.
  2. Flexibility in working hours may be needed to coordinate direct care and services with families and/or to provide outreach.
  3. Depending on assignment, work requires travel between programs and services.
