The ECM/CBAS Lead Care Manager is a pivotal professional role within the ECM & Community-Based Adult Services team, responsible for both overseeing the comprehensive care management and service delivery to the adult and elderly Members enrolled in the CBAS and ECM program and creating, monitoring and managing all referrals to the program. Will conduct outreach, enrollment, assessments and reassessments, developing individualized care plans, and provide ongoing care management. The role demands close coordination with the ADHC Center Program Director and involves the submission and management of referrals to the Enhanced Care Management (ECM) program for eligible individuals.

DIVISION: Enhanced Care Management Department

LOCATION: Santa Maria, CA

HIRING RATE: $26.00- $35.00/hourly

**Responsibilities**

**Key Duties**
- **Outreach and Enrollment**: Implement and oversee outreach strategies to identify and enroll eligible members into the Enhanced Care Management Program. Ensure the enrollment process is conducted efficiently, providing clear information to members about the services and benefits additionally, support the facilitation of the enrollment of ECM members who qualify for CBAS, into the CBAS program.
- **Assessment and Reassessment**: Conduct comprehensive initial assessments and periodic reassessments of ECM/CBAS members. Ensure that all required documentation is completed and submitted in a timely manner.
- **Care Planning**: Develop and maintain individualized care plans in collaboration with members, addressing the identified needs through a coordinated care approach.
- **Care Management and Coordination**: Provide consistent care management, acting as the primary point of contact for members enrolled in the ECM & CBAS programs. Coordinate with healthcare providers, community resources, and agencies to ensure seamless service delivery according to the care plans.
- **Collaboration with ADHC Center Program Director**: Maintain a collaborative relationship with the ADHC Center Program Director, updating them on Member outcomes, service delivery challenges, and the effectiveness of implemented care plans.
- **Documentation and Compliance**: Ensure comprehensive and timely documentation for each Member, including assessments, care plans, case notes, and communications, adhering to all regulatory and compliance standards.

- Bachelor’s degree in social work, psychology or related field, or equivalent experience.
- A minimum of two year of case management experience
- Leadership skills, and experience
- Excellent communication and interpersonal skills; proven ability to collaborate with community partners
- Availability to work occasional evenings and weekends.
- Experience working with mentally ill and dually diagnosed clients highly desirable
- An understanding of ideas and practices that promote diversity, equity and inclusion; and a commitment to ongoing professional development in this area.
- Bi-lingual Spanish language capabilities are highly desirable

**KNOWLEDGE AND SKILLS**
- Demonstrated knowledge of issues facing program participants (e.g. health, substance abuse, mental health, domestic abuse, trauma, immigration, legal)
- Ability to demonstrate empathy and a non-judgmental attitude when engaging with persons experiencing homelessness
- Possess a high level of tolerance and understanding for individuals who present for services with mental health and physical health needs
- Must be flexible and demonstrate ability to adapt and thrive in fast-paced and challenging environments. Lead Care Managers may be required to work across a variety of offices and field settings (motels, shelters, etc.)
