The Support Specialist for EFA (Emergency Financial Assistance) and HoME (Durable Medical Equipment) should be an individual with a passion for assisting vulnerable adults within the scope of JFCS’s mission and values. Programs include Emergency Financial Assistance and HoME-Durable Medical Equipment Program. This position works closely with Older Adult Services and is within our Community Services Department. The position provides services to clients including intake, compassionate communication, and resource provision, and works collaboratively with staff across JFCS to receive and respond to requests for assistance. The position participates in JFCS department and all staff meetings. The Support Specialist participates in education and marketing through community events and presentations.

**ESSENTIAL RESPONSIBILITIES**
- Complete intakes and follow up for financial assistance and durable medical equipment
- Provide case management services with a focus on financially vulnerable and low-income individuals and families
- Oversee and track multiple program budgets and contracts
- Ensure Electronic Medical Records are maintained according to agency policies and contract requirements
- Establish and strengthen working relationships with community agencies and Jewish community service agencies to meet community and client needs
- Communicate compassionately with clients in distressing situations, deescalating or referring the client as appropriate
- Participate in trainings, committees, and events
- Understands and accepts philosophy of agency

**WORK HOURS, SCHEDULE & FLEXIBILITY**

This position often works weekdays. There may be evening and weekends that will require your attendance in order to best represent the Agency. Some flexibility in scheduling exists, as long as agency needs are being adequately addressed.

**PROBLEM SOLVING AND JUDGEMENT**
- Follow established procedures while being creative and taking initiative
- Must be able to handle all facets of the business both standard, and non-standard situations

**REQUIREMENTS**
- Bachelor’s Degree or at least 4 years of relevant experience
- 3+ years experience providing services in the fields of gerontology, social work, nursing/healthcare, public health, personal finance, or other relevant areas
- Knowledge of community resources for vulnerable and at-risk individuals and families
- Case Management experience preferred
- Non-profit experience preferred
- Conversational Spanish-speaking a plus
- Ability to interact effectively in a team setting
- Ability to adhere to a trauma-informed care approach
- Ability to communicate effectively orally and in writing
- Ability to obtain a level 1 fingerprint clearance card, and pass a background check
- Proof of COVID-19 vaccine
