Job Summary

The position of Post Adoption Service Coordinator is part of a team supervised by the Director of Foster and Adoption Services. Post Adoption Service Coordinators are assigned a caseload of families who have previously adopted children/youth who were in the conservatorship of TDFPS and have experienced abuse, neglect, and/or exploitation. The position interacts with the family and service providers to reduce risk factors associated with adoption dissolution and promote stability within the family.

General Responsibilities

Accurately and timely complete family assessments.
Complete the necessary service request forms and authorize services for each client.
Contact clients and return all calls within 24 hours, or next business day (for non-emergencies).
Thoroughly and timely complete service plans with the family; ensure plans are updated following contract requirements.
Utilize family intake, assessment, and service planning information to create customized training for children and adults.
Develop and coordinate appropriate subcontracted services as needed.
Regularly oversee the familys progress to ensure family and child(ren) are making positive adjustments in order to maintain and stabilize the adoption.
Effectively coordinate the provision of all service request forms and authorize services for each client. Collect and provide documentation relevant to those services.
Complete accurate and timely documentation of each case management activity.
Respond to crisis intervention as needed and consistently teach, train, and support the family unit to achieve positive outcomes in strengthening the parent-child bond and avoiding permanent or long-term removal from the family.
Appropriately assign services for children to assist in coping with histories of abuse and neglect.
Effectively alleviate social, emotional, and financial stress for adoptive families through agency, community, and DFPS resources.
Report suspected child abuse or neglect, immediately to the Abuse and Neglect Hotline and the Director.
Effectively communicate with DFPS regarding the client and services provided.
Maintain regular and effective communication with Director about all incidents/crises.
Maintain regular and effective communication with the client, keep them informed and hold them accountable for the plan of service requirements.
Consistently work to develop the families in the areas of teaching and encouraging the use of therapeutic behavior interventions.
Consistently support families in organizational skills, advocacy and professional skills.
Preparing and completing all required post adoption paperwork accurately and timely.
Effectively assisting families in researching and applying for adoption resources and assist families with identifying therapists, day treatment providers, and residential treatment.
Open and process mail/e-mail daily in a timely manner.
Represent CK Family Services in the community by attending meetings and community events as requested.
Assist as staff in locating various training, camps, and enrichment events.
Attend all clinical meetings and department meetings or trainings as scheduled.
Maintain compliance with all CK Family Services policies, procedures, and regulatory requirements including all state and federal laws.
Participate in scheduled chart audits and ensure compliance measures for quality assurance.
Monthly supervision meetings with supervisor.
Communicate with clients and service providers professionally and positively.
Review and supervise maintenance of complete and current records on each client in compliance with contract and agency process.
Maintain confidentiality as required by government law and regulations.
Perform other duties and tasks as requested.

Qualifications:

Bachelor's degree in a human-services-related field, with 2 years of experience in foster care, adoption or a related field.
Candidates must have experience in service coordination and have a working knowledge of community resources.
Ability to motivate people; ability to develop and exercise good supervisory skills.
Ability to be on-call for families in crises.
Work deals mostly with areas such as preparing and reading data and figures and visual inspection involving small details. Visual work is primarily close to the observer. Depth perception and field of vision (peripheral) are important but not as critical as the ability to distinguish small details and markings very near to the observer.
Excellent interpersonal skills and demonstrated ability to effectively communicate orally and in writing with a variety of people; maintain confidentiality
Must have reliable transportation.
Ability to handle multiple tasks; self-starter; energetic; detail oriented.
Good character, personal attributes and mannerisms to be a good adult role model for residents and their families, and as a professional for case managers under supervision.
Ability work effectively under stress and remain flexible in changing situations.
Proficient in basic computer skills that include but are not limited to data entry, MS Office, jobsite portals and company software.
Ability to relate to multiples ideas and the concepts/theories behind the ideas; ability to remember multiple verbal and written tasks/assignments over several days and occasionally up to several months
Perform the physical demands of the position
Ability to use up to 100 pounds of force occasionally, up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people;
Ability to walk frequently; stand or sit for prolonged periods of time; squat occasionally, climb stairs infrequently, and kneel and twist as needed.
Sufficient good health to properly fulfill all role responsibilities. Employees shall not be permitted to work who have infectious diseases or skin lesions for the duration of the communicability.
Ability to perform moderately difficult manipulation such as writing, typing, foot and hand-eye coordination required.
