Role Overview:
* As a Regional Case Manager at Touchstone Communities, you will play a vital role in supporting the interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of facilitation and discharge planning functions
* 100% remote position. Orientation and initial training will be in San Antonio, TX
Who are we seeking?
* Current nursing license in the state required (RN or LVN)
* Must have at least 3+years of experience in skilled nursing/ long-term care setting and solid understanding and knowledge of obtaining authorization, discharge planning, utilization management, case management, performance improvement, and managed care reimbursement/ contract interpretation.
The ideal candidate will have experience in the following:
* Effective facilitation of precertification and payor authorization process.
* Effective facilitation of the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement.
* Monitor clinical documentation efforts and ensure that documentation accurately reflects severity of illness and intensity of servic
* Completes Utilization Management and Discharge Planning for assigned patients
* Maintains required documents relevant to managed care and discharge planning information according to standards.
* Coordination of the interdisciplinary plan of care
