Position Overview: This role offers a unique blend of field engagement and remote administrative duties. Case Managers will perform evaluations in the community four days a week while dedicating one day to documentation from a remote setting.
Key Responsibilities:
- Conduct comprehensive assessments in the homes of members across designated counties.
- Utilize personal transportation to reach member residences.
- Accurately document assessment findings and care plans.
Qualifications:
- Graduate from an accredited nursing program.
- Bachelor's degree in Nursing is preferred.
- A minimum of 2 years of clinical nursing experience.
- At least 1 year of experience in case management.
- Current RN license in the state of practice.
- Proficient in Microsoft Excel, Outlook, and Word.
Preferred Skills:
- Experience in home health care settings.
- Bilingual proficiency in Spanish and English.
- RUG certification is a plus.
- Experience with remote work environments.
