As the Facility Case Management Director, you're not just managing a department—you're orchestrating a symphony of care that spans across multiple teams, hospital departments, and external agencies, ensuring that every patient receives the attention and resources they need from admission to discharge.

In this role, you're the guiding force behind the scenes, ensuring that every aspect of case management runs smoothly. Your expertise as a registered nurse, combined with your leadership skills, makes you the ideal candidate to oversee the daily operations of the hospital's case management department.

You'll be the one ensuring that staff adhere to organizational policies and comply with external regulatory agencies. But more than that, you'll be integrating the patient care and discharge planning processes with other hospital departments, making sure that every patient's journey is seamless and their care is continuous. Your role is vital to optimizing clinical resources, ensuring that every patient receives the care they need without unnecessary delays or complications.

You'll work closely with the hospital's CFO, the Division Case Management Leader, and other key stakeholders to meet and exceed established goals.

As someone who's a self-starter, you'll have the freedom to manage multiple priorities, working on projects and activities with minimal supervision. You'll need to demonstrate tact and diplomacy, especially in situations of conflict or controversy, and be adaptable to the ever-changing demands of the healthcare environment.

Your responsibilities will include evaluating departmental operations, managing staffing needs, ensuring compliance with care coordination and discharge planning processes, and continuously assessing and improving the department's performance. You'll also build strong relationships with other hospital leaders, including the Chief Medical Officer, Chief Nursing Officer, and leaders of ancillary services.

If you hold a Bachelor's degree and have a Master's degree in Nursing, Health Administration, or Business Administration, that's a strong foundation for this role. Your experience in acute care hospital case management and leadership will give you the edge you need to excel in this position.
