This is a hybrid (remote-field) position. Care Managers will be working in the field 4 days per week doing assessments. 1 day per week will be working on documentation in remote home office. W ill need to be able to drive self in personal vehicle to the member homes. Travel will include the following counties: Dallas, Collin, Rockwall, Hunt, Navarro, Kaufmann, Ellis.
Pay: $38- $45 hourly, plus mileage reimbursement
Work Shift: Mon-Fri 8am-5pm, some overtime - sometimes mandatory
Education/Experience:
- Graduate from an Accredited School of Nursing.
- Bachelor's degree in Nursing preferred.
- 2+ years of clinical nursing experience in a clinical, acute care, or community setting.
- 1+ years of case management experience in a managed care setting.
- Licenses/Certifications: Current state's RN license.
- Computer literacy, Excel, Outlook, Word
Preferred Qualifications:
- Prior home health care experience
- Bilingual in Spanish and English
- Current RUG certified is preferred
- Remote experience preferred due to candidates recently having struggles working independently.
