DEPARTMENT SUMMARY: The Senior Services department supports and enriches the lives of lesbian, gay, bisexual and transgender (LGBT) people 50 years and older, with an overarching goal of successful aging in place. In this effort, the department provides a broad array of social, educational and support services to LGBT seniors, all of which are free or low-cost. We have an expanding population of LGBT homeless population and are growing as a department to include on site affordable housing for LGBT seniors.

JOB SUMMARY: Under the supervision of the Sr. Manager of Social Services and as part of the Seniors Services team, the Information & Engagement Coordinator will link LGBT seniors who need assistance with appropriate services offered at the Los Angeles LGBT Center Senior Services department and throughout Los Angeles County to resources and information. The ideal candidate must have background experience in community outreach, referral linkage, and experience in collaborating with external agencies and LGBT-friendly service providers. The Information & Engagement Coordinator will actively engage LGBT seniors at Center and community-based events within Los Angeles and promote programs and services offered at the Los Angeles LGBT center. Additionally, the Information & Engagement Coordinator will ensure the registration and orientation of new Los Angeles LGBT Senior Center participants and assist participants with navigating resources and referrals and provide administrative support to Department Assistants to ensure operation of the front desk.

ESSENTIAL FUNCTIONS:

1) Provides LGBT seniors with internal and external referrals to address identified needs and serve as an advocate for LGBT seniors with outside agencies through the following types of contacts; in person, through email and helpline/phone contacts, requires follow up and monitoring of clients to ensure information and referrals meet participant needs;

2) Assists clients with linkage to social service resources for seniors including, but not limited to: CalFresh; Medi-Cal/Medi-Care, insurance, affordable housing, mental health, healthcare, financial assistance, In-Home Supportive Services, Lifeline phones; legal and Veteran Affairs benefits

3) Conducts brief benefit screenings in order to address barriers to attaining services; and, providing support and guidance in navigating resources;

4) Refers clients to other staff members, other departments, and other agencies to access support and resources necessary; follows-up on the referral to ensure continuity of care;

5) Ensures appropriate documentation is entered into CIVI and other required data systems to meet all Department of Aging (DOA) and National Council on Aging (NCOA) requirements;

6) Maintains client-related data tracking systems, including case notes and entries in Civi CRM;

7) Conduct outreach and program orientation contacts to all new participants monthly.

8) Assists Department Assistants with responding to email and voicemail information requests and RSVP’s for events and activities.

9) Provide front desk coverage

10) Assist with serving meals to seniors

11) Assist in data compliance activities to ensure participants information is up to date and ID cards are printed

12) Assist with the development and management of participant matching software system

13) Offers clients the opportunity for regular feedback on services provided to measure effectiveness of services and interventions utilized;

14) Represent Los Angeles LGBT Center Senior Services at community-based and Center-based events to promote program and services available;

15) Conducts both outreach and in-reach activities, providing information of available services and connecting clients to resources. Community outreach will require some evening and weekend hours.

16) Prepares case-related reports including, but not limited to: outcomes, successes and challenges on a monthly basis (report template provided);

17) Maintains complete and accurate documentation of service objectives and outcomes for Department of Aging and other funding sources including but not limited to monthly tracking of outreach events and number of interactions at events with older adults

18) Participate in care management meetings, all-staff meetings, in-service trainings and other meetings, as assigned and in coordination of care support;

19) Facilitate informational groups for senior participants on public benefits, including research, developing a presentation and answering questions about rotating topics;

20) Facilitate support/discussion groups with senior participants to address both their social and emotional needs.

21) Assists with follow-up calls from clients; provide information and referrals to assistance as appropriate.

22) Other duties as assigned;

JOB QUALIFICATIONS AND EXPERIENCE:

1) Knowledge of or experience working with the LGBTQ community and familiarity with issues of particular relevance to LGBTQ people;

2) A passion for the Center’s work and its mission to make the world a better place for LGBTQ people;

3) Experience in care management and community-based service delivery, required;

4) A minimum of 2 years of experience in a social service or related setting

5) Experience maintaining confidential information, understanding of Health Insurance Portability and Accountability Act (HIPAA) requirements, and pass HIPAA certification training for HIPAA compliance (within 90 days);

6) Completion of Serv Safe Food Production Manager Certificate within 90 days.

7) Working knowledge of issues impacting LGBT seniors in ethnically and socio-economically diverse urban and suburban environments;

8) Knowledge of social service resources for seniors including, but not limited to: Medi-Cal/Medi-Care, affordable housing, mental health, healthcare, legal and Veteran Affairs benefits, In Home Supportive Services;

9) Bilingual skills, a plus (English/Spanish fluency preferred; American Sign Language, a plus);

10) Demonstrated ability to manage multiple tasks under pressure in a fast-paced environment;

11) Excellent verbal, written, time management, interpersonal, research and computer skills (Microsoft Word and Excel);

12) Able to work alternative work schedules, evening and weekends as scheduled;

13) Ability to regularly travel to outreach events with access to reliable transportation and the ability to be insured;

14) Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment.
