We are seeking a compassionate and dedicated Individuals to join our team as a Community Resource Case Manager.


AWARE’s PACT team offers a wraparound approach to mental health care. Our team provides medication management, individual and group therapy, substance use therapy, vocational rehabilitation services, community resource services, peer support, and nursing services. We provide clients the services they need to ensure they are able to manage their mental illness in a community setting.

Principal Duties & Responsibilities:

  • Ability to, under AWARE’s Unconditional Care Philosophy, develop and implement individual treatment plans with the knowledge of behavioral, clinical, and mental health interventions, skill building techniques, and understanding of the emotional and behavioral needs of assigned clients.
  • One-year experience in mental health services or a related field.
  • Interpersonal oral and written communications skills to work as a team member, to make observations and to report them accurately, to learn behavioral, clinical, and mental health care methods, and to follow oral and written instructions and have the ability to facilitate meetings.

Requirements:

Talents, skills, and abilities:

  • High School Diploma or HSE is required, along with past employment experience.
  • BS/BA Degree in a social work-related field and minimum of one (1) year of direct work experience in the human services field.
