Reporting to: VP of Business Development


Job Summary: As a Community Outreach Associate at True Care, you will play a pivotal role in fostering meaningful connections within the community. This position centers on actively engaging with diverse groups, forming robust partnerships, and amplifying True Care's presence through innovative outreach strategies. Your responsibilities will include generating referrals, orchestrating impactful marketing campaigns, and representing the company at various community events. In this role, you will be instrumental in enhancing True Care's visibility and reputation as a leader in home healthcare, directly contributing to the growth and outreach success of the organization.


Responsibilities: 


  • Referral Generation: Actively seeking and securing referrals to expand True Care's client base.
  • Partnership Development: establish and maintain strong relationships with community organizations and stakeholders, focusing on cultivating referral sources and improving patient census through the start of care for services.
  • Strategic Relationship Building: Fostering and nurturing strategically important relationships with an emphasis on long-term community engagement and partnership value.
  • Marketing Initiatives: Creating and executing innovative marketing strategies to enhance True Care's visibility.
  • Event Participation: Representing True Care at community events, showcasing the company's commitment and services.
  • CRM Management: Maintaining accurate records of outreach activities and communications in Salesforce, ensuring data accuracy and timely updates.
  • Other Duties: Performing additional tasks as assigned and adapting to evolving organizational needs.


Qualifications: 


  • A Bachelor's degree in a relevant field.
  • Proven experience in community outreach, marketing, or a similar role, preferably in the healthcare sector.
  • Exceptional verbal and written communication abilities.
  • Strong organizational and time management skills.
  • Demonstrated leadership capabilities.
  • Proficiency in Microsoft Office Suite and Salesforce.

Valid driver's license for community engagement purposes


KPI’s: 

In this role, success will be measured through a variety of performance indicators aligned with our company's objectives and operational standards. Candidates should be prepared to 


engage in goal-oriented work and be evaluated against clear, quantifiable benchmarks to ensure continuous improvement and effectiveness in the position.

Key Performance Indicators


  1. Referral Generation Efficiency: Target of 15-20 increase in new referrals per month.
  2. Community Engagement Impact: Aim to engage with 4 community members or organizations monthly, with a 25% increase in event attendance or participation.
  3. Partnership Development: Establish and sustain at least 3 new strategic partnerships per quarter.
  4. CRM Data Management: Maintain 100% accuracy in Salesforce entries, with all data updated within 48 hours of interaction.
  5. Start-of-Care Processes: Achieve a target of 7 successful start-of-care instances each month.
