  • Experience with Maryland Medicaid Incedo and SUD Authorizations.
  • Provide intake assessments and coordinates the referral and intake process, and maintain communication with referral sources, families, patients, and staff.
  • Respond promptly to patient needs and redirecting clients as necessary.
  • Schedule clients for counseling sessions, initial intake appointments, and manage authorizations.
  • Answer telephones and be the direct contact person for admissions.
  • Work in cooperation with Clinical Director, Medical Director, colleagues, and Operations staff.
  • Complete assigned work in a timely fashion.
  • Adapts to new or unusual situations without affecting work performance or other staff negatively.
  • Maintains pleasant and professional attitude and appearance.
  • Performs other related duties as required.
  • Provide crisis intervention when necessary.
  • Document client intake information in EHR.
  • Adhere to HIPAA and other state and federal confidentiality laws.
  • Establish and maintain productive effective communications with clients, family members and staff.
  • Deal with team members regarding problems in a constructive manner.
  • Accept and use direction and supervision.
  • Ability to effectively present information in one-on-one and small group situations to community members and employees of the organization.
  • Upon admission, complete a case management assessment with client to determine if there is a need for housing for aftercare placement, insurance renewal, electronic benefit transfer, and if there are any pending legal issues. Will also obtain ROI for referent, family, EAP and/or employer. Ensure client has notified family of arrival.
  • Produce all documentation needed for the client's employer, legal, etc. including, but limited to, FMLA, STD, LTD, Treatment Letters, etc.
  • Meet with each client once weekly, at a minimum, for case management appointments.
  • Develop a strong, ongoing relationship with clients and their families. Maintain ongoing healthy communication with the clients families.
  • Develop effective aftercare plans for all clients. Ensure that all clients follow their establish aftercare plan. Work with the outreach and clinical team to determine an appropriate aftercare plan.
  • Update and modify ALL the client's discharge dates, which are determined by the Clinical and medical team.
  • Accurately share information in team meetings.
  • Ensure medical records are sent to the facility where a client is discharging to.
  • Maintain a basic understanding of health insurance benefits, health insurance terms, and other payment methods that are used for our organization.
  • Remind clients of the program rules and regulations as necessary and report infractions appropriately.
  • Interact appropriately with clients, showing them respect while maintaining boundaries.
  • Protect the privacy of all client information in accordance with the organization's privacy policies, procedures, and practices, as required by federal and state law, and in accordance with general principle of professionalism as a health care provider.
