Job Summary:
The Behavioral Health Care Coordinator is a core member of the care team, including the patient’s medical provider and psychiatric consultant, as well as the larger primary care team. The Behavioral Health Care Coordinator is responsible for supporting and coordinating the mental and physical health of patients on an assigned patient caseload with the patient’s medical provider and, when appropriate, other mental health providers.
Essential Functions:
- Communicates to patients the purposes of the program and the impact it may have on their wellbeing. Helps patients identify social determinants of health that affect their overall health and develop health and social management plans and goals.
- Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers
- Completes intake and check out of patients, including but limited to vitals, point of care, and medication administration.
- Provides and explains discharge instructions and patient education. Allowing patients the opportunity to have any questions or concerns answered prior to leaving their appointments.
- Coordinates and provides access to individual and family supports, including referral to community social supports.
- Complete Prior Authorizations for medications and services to assist with care coordination.
- Coaches/encourages patients in the self-management of their health issues; providing resources and education as necessary.
- Provides education on health conditions and strategies to implement care plan goals including both clinical and non-clinical.
- Schedules and follows up on behavioral health referrals to increase access of continuous care.
- Oversees and coordinates Vivitrol treatments and compliance for patients enrolled in the program.
- Collaborates with PCP, patient, and members of the health care team, including continuum of care settings and community. Responsible for developing a comprehensive individualized plan of care and targeted interventions incorporating the patient’s preferences and goals.
- Provides follow-up to patients to find out if they are meeting their goals and how they are managing their health issues; celebrates successes and provides suggestions about how to overcome barriers.
- Effectively communicates with the patient’s Care Team/PCP, to exchange information about the patient, to gain insight into the patient’s clinical challenges and to give updates on progress made with Care Management and Self-Management Support services.
- Maintains HIPAA standards and confidentiality of protected health information,
- Performs other duties as assigned.
