Position: Care Coordinator

Shift: Mon-Fri 8:00 am to 5:00 pm

Salary Range: $56,804.80 annually - $73,444.80 annually. We use a bona fide factor system to determine starting wage (years of experience, education, credentials, and specialty skills that relate to the job requirements).

Position Requirements: requires a QMHA: a bachelor's degree in a behavioral science field from an accredited college or university. Note: All QMHA positions in the state of Oregon will need to be filled by candidates who are officially QMHA certified or registered with MHACBO. For details, go directly to MHACBO (www.mhacbo.org)

Summary: The Counselor I functions collaboratively as part the Outpatient Behavioral Health team. This team addresses behavioral health needs including but not limited to significant emotional dysregulation, disordered thought processes, substance use intoxication resulting in behavioral crisis and environmentally de-stabilizing events that require multi-disciplinary crisis intervention. As a The Counselor I will provide compassionate, person-centered, trauma-informed care to individuals with the goal of stabilization at the least restrictive level of care and an emphasis on post crisis growth. An ideal candidate to fill this Counselor I position will work with community partners, stakeholders, and the multidisciplinary team to coordinate client care and safety. The successful candidate will have experience in working with individuals with psychiatric diagnoses or symptoms, as well as knowledge in assessing and triaging, medication administration and crisis management skills.
