Job Title: Integrated Mental Health Support Specialist



Job Description:


The Integrated Mental Health Support Specialist provides comprehensive support, counseling, and coordination services to an adult population with Mental Health, Substance Use Disorder and/or HIV-AIDS related disabilities. This role combines the responsibilities of a residential counselor and a case manager, advocating for and responding to clients' treatment, rehabilitation, and housing needs. Services are provided from a social rehabilitation philosophy, focusing on teaching daily living skills in a safe and healthy environment.


Key Responsibilities:

  • Client Assessment and Planning:
  1. Conduct intake interviews and assist with developing, implementing, and planning client treatment plans.
  2. Assess clients for housing eligibility, clinical needs, and potential crisis situations, determining appropriate interventions.
  3. Prepare Income Strategy Plans and Housing Benefits Plans with clients, supporting them in implementation.
  • Counseling and Support:
  1. Conduct individual and group counseling sessions, facilitating outpatient Medi-Cal billable groups, Facilitate and document weekly house meetings and individual counseling sessions with all clients.
  2. Act as a role model, guiding clients in daily living skills, self-care, personal interaction, social relationships, and constructive time management.
  • Crisis Intervention and Monitoring:
  1. Evaluate clients for potential crisis situations and determine appropriate interventions.
  2. Monitor and document clients' progress toward achieving individual goals, maintaining detailed and timely documentation in client charts.
  • Resource Referral Support:
  1. Maintaining external and internal resource guides for housing, work force development, medical and community service providers.
  2. In Transitional Residential Treatment Programs, emphasize the development of vocational skills and link clients to services offering transitional employment or job placement.
  3. Ensure clients are linked with primary care providers and assist with identifying and accessing community resources.
  • Daily Operations and Supervision:
  1. Primary supervision of clients, facilitating and supervising house chores, meal preparation, and group activities.
  2. Ensure clients maintain their environment in a safe and sanitary condition, reporting any property problems to the Housing and Benefits Manager.
  3. Participate in staff and shift change meetings, maintaining professional boundaries with clients and staff.
  • Structured Services:

Provide structured day and evening services to include;

  • Individual and group counseling
  • Crisis intervention
  • Planned activities
  • Counseling with family members when indicated in the client's treatment plan
  • Development of community support systems to maximize clients' use of non-mental health community resources
  • Pre-vocational or vocational counseling
  • Client advocacy, helping clients develop their own advocacy skills
  • Activity programs encouraging socialization within the program and the general community, linking clients to resources available after leaving the program
  • Use of the residential environment to assist clients in acquiring, testing, and refining community living and interpersonal skills

Compliance and Training:

  • Comply with agency policies, procedures, workflows, and documentation requirements.
  • Maintain the agency's staff training requirements.
  • Obtain First Aid and CPR certifications, TB Test, upon hire and maintain these certifications during employment

Minimum Qualifications:

  • High school diploma or GED with a minimum of two (2) year of full-time experience, or its part-time equivalent, in direct provision of services to clients with mental disabilities.
  • Strong knowledge of issues faced by adults with substance abuse, domestic violence, HIV/AIDS, mental health issues, history of trauma, and co-occurring diagnoses.
  • Knowledge of harm reduction strategies and experience working with government agencies, nonprofit organizations, and community stakeholders.
  • Bilingual (English/Spanish) highly desirable.
  • Strong organizational skills with attention to detail and accuracy.
  • Ability to work independently with minimal supervision and prioritize diverse tasks.
  • Basic understanding of general computer skills and practices; proficiency in Microsoft Office 360 and EPIC, preferred.
  • Ability to work flexible schedules/shifts/areas and at a San Francisco worksite post-Covid.
  • DOJ/FBI and California Department of Social Services (CDSS) background clearance required for Grove Street House.
