The caseload is managed through referrals from Behavioral Health Services, with a limit of 16 clients to facilitate the delivery of intensive care. Once clients achieve stability, they are transitioned to Geriatric Outpatient Services. The Geriatric Integrated Care Specialist evaluates client needs, collaborates with the medication support team to create tailored treatment plans, and provides services such as therapy, crisis intervention, advocacy, prevention, and education. Meticulous documentation is maintained for billing purposes, ensuring compliance and accountability.
Qualifications:
- AMFT/ ASW/ APCC certification
- Practical experience in case management and mental health care for at-risk older adults, particularly those with co-occurring disorders
