Hospice of the Piedmont is seeking a full-time Director of Performance Improvement. Reporting directly to the CCO, the Director of Performance Improvement will be responsible for ensuring multi-disciplinary quality of care and services through performance improvement activities across all services of Hospice of the Piedmont. This role makes significant contributions to operational improvement and efficiency by working with and alongside HOP leadership and teams, setting the tone for high standards and mutual accountability, and addressing deficiencies in performance through strong collaboration with HOP Senior Leadership, Education, and Care Managers.


Responsibilities/General Duties (Including but not limited to)

  • Strategic Initiatives/Goals Development: Develop strategic initiatives and goals related to the Quality program at Hospice, including patient and caregiver experience and all interdisciplinary services to improve clinical and key performance initiatives
  • Leadership Support: Provide and support leadership across the organization to maximize program efficiencies.
  • Compliance Assurance: Work with key stakeholders to ensure compliance with all Federal and State Agencies
  • Performance Deficit Correction: Develop and evaluate action plans for correcting clinical performance deficits, including quality assurance, performance improvement, and standards of practice.
  • Education and Competency Improvement: Collaborate with HOP Education and leadership to identify education needs and competency opportunities to improve quality, safety, and service.
  • New Employee Orientation and Onboarding: Support new employee orientation and onboarding to quality goals and service standards.
  • Complaint and Incident Investigation: Work with the Director of Quality to investigate complaints or incidents involving quality, safety, and/or dissatisfaction with hospice services.
  • Clinical Team Scheduling: Collaborate with HOP clinical leadership to evaluate and maximize scheduling of clinical teams to ensure compliance with the plan of care and maximize efficiency.
  • Technology Utilization: Work with key stakeholders to maximize the use of available technology such as TapCloud and Celltrak to improve organizational performance.
  • Innovation and Best Practices: Generate innovative ideas and apply industry best practices to ensure outcomes meet or exceed expectations.
  • Coaching and Mentoring: Work with HOP team members and management to coach, mentor, and guide them to optimum performance.
Requirements:

Qualifications:

  • Education: BSN and/or Healthcare degree required; Master’s level preferred.
  • Experience: More than 5 years of hospice experience. Experience in process and quality improvement preferred.
  • Skills & Abilities: Proficiency in writing, critical thinking, systems and data analysis, Microsoft Suite, and Tableau. Strong communication skills and complex problem-solving skills.
