FLSA: Full-time (40 hours/week), Non-exempt (hourly) plus benefits

Schedule: Monday – Friday, 6:30 am to 3:00 pm

Salary: $29.38 per hour

Location: Downtown San Diego area

Position Summary

Under the direction of the Program Manager of Unhoused Care Team, the Lead Case Manager plays a dual role, serving as a team leader while also maintaining a caseload of clients. This position requires leadership, coordination, and direct engagement with individuals experiencing homelessness, providing guidance, support, and resources while leading the outreach team towards impactful community engagement.

Essential Job Functions & Duties (75%)

  • Supervise, mentor, and support the Case Managers while effectively managing their own caseload, ensuring a balance between leadership duties and direct client engagement.

  • Maintain a caseload of clients, conduct comprehensive assessments, develop individualized service plans, and provide ongoing support to ensure access to necessary resources such as housing, healthcare, and social services.
  • Oversee and coordinate outreach activities, ensuring effective coverage, resource allocation, and collaboration among team members to optimize engagement.
  • Oversee data collection, documentation, and reporting related to outreach activities and caseload management, ensuring accuracy and compliance with reporting requirements.
  • Conduct regular audits and checks to ensure data integrity, consistency, and compliance with reporting requirements.
  • Maintain and develop relationships with federal, state, and local agencies that serve as referral resources for clients.
  • Perform special assignments unique to the department’s function (homeless counts, data processing, presentations etc.)
  • Perform other tasks as assigned by department, including Executive Director/Senior Vice President of the Clean & Safe program.

Financial Management (10%)

  • Receive verbal/written approval from Program Manager of Unhoused Care Team for all purchases.

  • Track client incentives accurately for requests for reimbursements
  • Complete purchases in a timely manner, scan all receipts to Accounting within 24 hrs. of purchase, placing physical receipts in finance inbox.

Administrative Management (15%)

  • Assist in coordinating projects by collaborating with different teams, tracking project progress, ensuring deadlines are met, and compiling reports or documentation as needed.
  • Assist in maintaining compliance with relevant regulations and company policies. Manage documentation and records, ensuring they are organized and easily retrievable.
  • Adhere to all local, state, and federal guidelines and laws. (i.e., HIPAA, OSHA, FLSA)
  • Answer all incoming calls from clients and various providers in a polite and professional manner.
  • Respond to all email requests within 72 hours.

Required Qualifications, Skills, and Abilities

  • Understanding of the homeless population, their needs, and available resources
  • Excellent leadership, interpersonal, and case management skills, with the ability to motivate and guide a team effectively while managing a caseload.
  • Prior experience working with HMIS or similar databases in a social services or nonprofit setting.
  • Proficiency in data entry, database management, and reporting tools.
  • Excellent communication skills and ability to train and collaborate with diverse teams.
  • Basic knowledge of Microsoft Word, Excel, PowerPoint, and Outlook
  • High School Diploma or GED (AA or BA preferred)
  • Ability to read and write in English (bilingual or multilingual preferred)
  • Valid Driver’s License required
