Position Summary:

The Program Coordinator has demonstrated clinical skills in assessing and managing the care of persons with Alzheimer's and related dementia. The Program Coordinator acts as a liaison of the Living Moments Dementia Unit in the center with other departments, the dementia community, and the residents and families that the unit serves. The Program Coordinator supervises the supportive care and daily routines of the residents and ensures that their emotional, social, and physical needs are met as best feasible. This position works exclusively in the unit and follows all center standards and procedures.


Essential Functions:

  • Participates in training programs and assists in orientation of new staff
  • Assists with pre-assessment of potential residents
  • Oversees completion of a family/primary caregiver assessment, interprets the assessment, and facilitates a Plan of Care for that individual to best integrate their involvement with the resident.
  • Monitors and adjusts the daily living routine on the unit to best meet the resident's needs.
  • Manages the clinical assessment of the residents through the Interdisciplinary Plan of Care (IPOC) process integrating the services of the center as needed.
  • Provides community education and related services to those interested locations where dementia services are relevant.
  • Participate in marketing events.
  • Participate in care conferences
  • Follow direction/instruction/delegation given by RCM assigned to unit

Experience:

Have at least three years' experience providing care for individuals with Alzheimer's or related dementia and 2. Have advanced practice knowledge in aging, Alzheimer's or other related dementia.
