The Care Coordinator (CC) is primarily responsible for providing administrative support to clinical staff across the multitude of services, programs and initiatives at our community health center with the end goal of closing care gaps. In addition, they serve as the patient’s ultimate advocate between the health center and outside entities.


The Care Coordinator communicates with primary care providers, referral providers, the patient/caregiver, and family to assist with the receipt of needed services, ensure communication across the care team and reinforce patient adherence with the care plan. The Care Coordinator works collaboratively in pre-visit preparation, working with care team during and after the patient visit and population health activities. They facilitates patient appointments, including those made with labs, diagnostic areas, and specialty physicians.


Patient Program Coordination

  • Pre- Visit Preparation
  • Facilitate patient appointments, including those made with labs, diagnostic areas, and specialty physicians where special assistance is needed.
  • Follow-up with referral provider to obtain reports on labs and diagnostic tests and consultant notes as appropriate. Follow-up with patient/family if test was not done or appointment was not kept and facilitate receipt of test or provider visit.
  • Participate in team huddle prior to clinic session to review needed care plans and anticipate flow.
  • During Patient Visit
  • Review prior care plan goals and progress towards goals with patient/family. Update care plan as appropriate.
  • Reinforce care plan with the patient/family in the case of initial visit or initial care plan development.
  • Facilitate enrollment application process and connect them with enrollment specialist as needed.
  • Between Patient Visits
  • Notify patients of referral status updates.
  • Update referral status to ensure proper closure of loop within EHR system, eClinicalWorks.
  • Identify gaps in care for designated patients and facilitate coordination of needed services.
  • Other Duties
  • Communicates frequently with the care team regarding patient requests, scheduling issues or other aspects of duties.
  • Effectively maintains and protects the confidentiality of all medical records and patient information.
  • Maintain accuracy of patient demographics and encounters.
  • Perform other duties as assigned.
  • General Agency Duties
  • Fosters an environment that promotes multidisciplinary team care consisting of trust and cooperation among all staff.
  • Enforces clinic policies and procedures.
  • Maintains confidentiality of all patient and employee information to all except designated employees.

Requirements:

KNOWLEDGE, SKILLS, AND ABILITIES

  • Excellent verbal and written communication skills.
  • High level of skill in maintaining calm, professional, courteous and helpful demeanor in times of stress.
  • Skill in making appropriate decisions to benefit patients and meet company objectives.
  • Ability to prioritize work and complete it on a timely basis with minimal supervision.
  • Ability to follow procedures.
  • Ability to deal with change and seek out opportunities to effect change to promote patient care.
  • Strong customer service approach to problem solving situations.
  • Ability to read, write, understand and spell English and Spanish (or other patient language needed) correctly.

EDUCATION & EXPERIENCE REQUIREMENTS

  • High school graduation or equivalent.
  • Experience working with under served and diverse populations.
  • Fluent in Spanish

PREFERRED EXPERIENCE

  • Experience working with common health care programs.
  • Experience working with patient registration procedures.
  • Basic knowledge of medical terminology.
