About Us: Meals on Wheels Orange County is the largest nonprofit provider of nutrition and supportive services for at-risk older adults in Central and North Orange County. Our mission is to combat hunger and loneliness among older adults through nutritious meals, friendly visits, and safety checks, and to support families through day services. We deliver nearly 2 million meals annually to 20,000 at-risk older adults.

Location: Santa Ana, CA

Hours: Monday - Friday, 8:00 AM - 4:30 PM

Hourly Range: $18.50 - $23.00 per hour (Hourly rate will be determined based on factors such as geographic location, skills, education, and experience)


Position Overview: We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator will ensure patient navigation by managing client caseloads, conducting intake assessments and reassessments, and facilitating communication between interdisciplinary Care Teams. The role involves supporting members, their families/caregivers, and community resources, aiming to provide seamless and coordinated care.

Key Responsibilities:

  • Coordinate with individuals and entities to ensure a seamless experience for members and prevent duplication of services.
  • Increase continuity of care by managing relationships with tertiary care providers, transitions-in-care, and referrals.
  • Screen clients for eligibility for direct and support services, and refer them to needed services, such as mental health, housing, crisis, and employment assistance.
  • Conduct client-specific needs assessments, identify problems, and establish immediate requirements and long-range goals.
  • Arrange and coordinate a network of supportive services and entitlements (both formal and informal) consistent with mutually developed care plans.
  • Maintain required records and reports in compliance with department, agency, local, state, and federal requirements.
  • Schedule and attend meetings to provide program information.
  • Assume responsibility for all case records and monthly statistics.
  • Meet program targets and participate in all mandatory training sessions and meetings (including CPR and First Aid training) as required by state regulations.
  • Complete home visits and meet with patients in their homes.
  • Coordinate with hospital and SNF staff on discharge plans.
  • Develop and coordinate monthly schedules for transportation needs with the transportation provider, Supportive Services team, and residents.
  • Administer transportation registration, including maintaining registration lists, attendance records, and compliance documentation, and provide information to appropriate partners.
  • Accompany residents on scheduled trips to ensure their safety and well-being.
  • Connect members to other social services and supports as needed, including transportation.
  • Perform other duties and special projects as assigned.

Qualifications:

  • Medical assistant certification preferred or related educational certification.
  • Experience in case management is preferred.
  • Experience working with older adults, the elderly, and people with disabilities.
  • Excellent communication, written, and interpersonal skills.
  • Thorough knowledge of case management principles and techniques.
  • Ability to maintain professional and confidential standards in client business-related activities.
  • Demonstrates a “can-do” spirit, a sense of optimism, and commitment.
  • Strong problem-solving and critical thinking skills required.
  • Working knowledge of programs and services available in Orange County for seniors.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
  • Strong understanding of cultural competency with the target population.
  • Must pass a background check.
  • Driver’s License and proof of Insurance.
  • Bilingual in Spanish is required.

Physical Job Requirements:

  • This position operates in a fast-paced, multi-tasking, professional environment, requiring ongoing learning and complex problem-solving.
  • Ability to sit for extended periods and use both hands and fingers for typing.
  • Ability to operate office equipment such as copiers, printers, filing cabinets, and telephones.
  • Occasionally lift and move office equipment and materials up to 10 pounds.
  • Frequent computer use requires close vision.
  • The noise level in the work environment is usually moderate.
  • Occasional overtime required.
  • Local travel required up to 50% of the time.
