Job Summary

We are seeking a skilled Clinical Care Coordinator to join our team at Christus Health. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of high-quality patient care.

Key Responsibilities
  • Collaborate with physicians, staff, and other healthcare professionals to provide a seamless healthcare continuum for patients.
  • Work closely with the clinic care team to ensure safety, best practices, and high-quality standards of care are maintained.
  • Coordinate patient wellness and chronic illness activities for the entire patient population.
  • Measure success through outcome performance measures, such as patient satisfaction, ACO, HCC, and clinic quality dashboards.
  • Act in a clinical leadership role, utilizing critical thinking and creative problem-solving to ensure positive outcomes in clinical care delivery.
Requirements
  • Graduate of an accredited program for Registered Nursing (RN) required; BSN preferred.
  • Licensed by the State of New Mexico as a Registered Nurse.
  • BLS Certification issued through the American Heart Association required, or within 2 weeks of hire date.
  • Knowledge of good business English, spelling, arithmetic, and practices.
  • Effective communication and teaching skills, with the ability to coordinate and implement educational materials for patients and their support systems.
  • One year of experience working as a nurse in a clinical setting preferred.
Working Conditions
  • Responsible to: CSVCG Dyad
  • Supervises daily activities of medical assistants.
  • Ensures timely orientation and competencies completion for all clinical support staff.
  • Provides feedback to manager for annual performance evaluations of clinical staff.
Physical Requirements
  • Requires full range of body motion, including handling and lifting patients, manual and finger dexterity, and eye-hand coordination.
  • Requires standing and walking for extensive periods of time.
  • Occasionally lifts and carries items weighing up to 50 pounds.
  • Frequent exposure to bodily fluids, communicable diseases, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinic environment.
