Position Overview

SUMMARY: Under the guidance of the Clinical Manager/Director, the RN Assessment / MDS Coordinator plays a crucial role in overseeing and facilitating the completion of comprehensive resident assessments. This includes the Minimum Data Set (MDS) and ensuring the accurate tracking and submission of all relevant data, including RUG levels, to the Centers for Medicare & Medicaid Services (CMS). The role necessitates collaboration with a multidisciplinary team of health professionals to conduct thorough initial and periodic assessments of residents, aimed at devising care plans that support residents in achieving their optimal levels of physical, mental, and psychosocial well-being.

KEY RESPONSIBILITIES:

  • Engage in departmental Performance Improvement initiatives.
  • Exhibit knowledge of Enhanced Standard Precautions and Isolation protocols.
  • Serve as a backup for the unit's Clinical Manager/Director.
  • Collaborate effectively with nursing staff, physicians, consultants, department heads, residents, and their families or legal representatives to address resident needs.

QUALIFICATIONS:

  • Minimum Experience: Preferred 6 months in a similar role.
  • Required Licenses/Certifications: Valid CA RN license, BLS certification, and MAB/ABM certification (to be obtained within 90 days of hire). Additional certifications may be required for specific units (e.g., ER - ACLS, PALS & CPI).
  • Ability to fulfill essential job functions with or without reasonable accommodation.
  • Adhere to all applicable state and federal regulations, as well as hospital policies and procedures.
  • Demonstrate accountability and ownership of responsibilities.
  • Complete mandatory training and compliance requirements, including annual competency assessments and TB screenings.
  • Successfully meet the Company's compliance competency standards.
