Job Overview:
The role involves liaising with primary care providers to communicate about patients being discharged via fax.
Key Responsibilities:
- Contact primary care providers to secure necessary orders.
- Ensure all new admissions are processed daily and appropriately staffed.
- Complete daily recertifications, assessing their necessity at the end of each episode, and manage staffing through assistants.
- Respond to all email communications promptly.
- Request necessary authorizations for patient care.
- Maintain knowledge of DSL and locate patient information, including primary care providers and orders.
- Oversee the completion of lab tests, ensuring results are documented in patient charts and communicated to the ordering physician, primary care provider, and pharmacy when needed.
- Schedule visits and manage documentation in the absence of assistants.
- Verify the accuracy of lab forms.
- Collaborate with hospitals to verify information and update quality assurance records.
- Ensure that all required care plans, fall reports, and incident reports are completed as necessary.
- Create orders for other healthcare professionals.
- Contact primary care providers when there are changes in patient conditions.
- Understand and navigate workflow processes effectively.
- Possess a thorough understanding of various insurance plans.
- Submit reports for Regal cases bi-weekly.
- Participate in case manager meetings.
- Complete any additional tasks as assigned by management.
Requirements: Must be a registered LVN or possess over two years of relevant experience in the healthcare field.
