We are looking for a knowledgeable, professional individual to be a Service Coordinator for our Preschool Program. Service Coordination or case management experience desired. Hours: Monday-Friday 7:30a-4:00p

Job Summary: The Service Coordinator works under the direct supervision of the Service Coordination Supervisor, initiates and maintains the referral/intake/admissions process, assists guardians with the admissions and service process, insures that Medicaid eligibility of all consumers are kept current, serves as the liaison to the billing, teaching, and nursing departments, assists in the creation and maintenance of accurate and current admissions records, develops consumer service plans, assists with the establishment and maintenance of all consumer files, assists classroom and certified staff with coordination of consumer services.

Essential Functions and Responsibilities include the following, other duties may be assigned.

  • Receives all referrals for admission for assigned age group.
  • Maintains current knowledge, through off site training(s), regarding all admission procedures for Early Intervention and Early Childhood.
  • Responsible for the coordination of interdisciplinary team meetings.
  • Assist with coordinating therapy evaluations and medical examinations.
  • Assists families with completing reports from other agencies, as needed.
  • Assists guardians in completing and compiling necessary paperwork for admission and securing a funding source for services. Also, assures the continuation of funding sources for all services.
  • As necessary, coordinates the services of the evaluators, social agencies physicians, etc.
  • Maintains a positive working relationship with DDS, DHS, EI, Part C, DCFS and DCC voucher staff.
  • Maintains accurate and up-to date records on funding eligibility for all assigned consumers.
  • Provides quality services to families and children in both the facility and community settings.

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Education and/or Experience: Bachelor's degree in Education, Social Work, or a related field, or DDS Certification as a Case Manager. In addition, documentation of two years of previous experience in working with individuals with developmental disabilities and documentation of completion of the First Connections Early Intervention Service Coordination Block Courses (30 hours) with a minimum of 70% proficiency.

Certificates, Licenses, Registrations: Service Coordination Certification, CPR/First Aid, Arkansas Driver's License and Auto Liability Insurance.

Computer Skills: To perform this job successfully, an individual should have knowledge of Database software; Excel Spreadsheet software and Microsoft Word, Word Processing software.

Safety Sensitive Position: This is a safety sensitive position as it involves operating a motor vehicle as part of the job duties and a lapse of attention could result in injury, illness, or death to the consumers being transported. See Ark. Const. Amend. 98 2(25). All positions classified as safety must refrain from reporting to work or being subject to duty while their ability to perform job duties is impaired due to the use of alcohol or other drugs. All positions classified as safety sensitive are prohibited from testing positive for metabolites of substances that are listed in the schedules of the Federal Controlled Substances Act and the Arkansas Uniform Controlled Substances Act, regardless of whether the employee possesses a medical marijuana registry identification card. See id. at 3(f)(3)(B)(iii). This policy applies during all work hours, whenever conducting business for AEDD.
