POSITION SUMMARY:

The Coordinator, Special Projects plays a critical role in achieving the objectives related to several initiatives that inform long-term design of client-centered nutrition care at Project Open Hand. This role’s primary responsibilities include a blend of administrative and direct service duties. Specifically, this role will (a) monitor all day-to-day activities for the planning and implementation of a suicide prevention training program funded by a 3-year grant from the Administration for Community Living (ACL), and (b) provide care navigation for participants in a study conducted in partnership with the University of California, San Francisco and funded by the American Heart Association (AHA) Health Care by Food initiative. This role will support the needs of contracts awarded through April 30, 2026, with the possibility of extension or expansion based on new initiatives.

QUALIFICATIONS REQUIRED:

  • Experience working in a community-based and/or non-profit setting in support of diverse populations
  • At least 2 years of experience in project and program coordination
  • Proficient in MS Office, using databases, and internet research
  • Demonstrated ability to maintain professional boundaries in a social service setting
  • Flexible schedule with the ability to work in POH’s San Francisco office, as needed

QUALIFICATIONS DESIRED:

  • Bachelor's degree in human service or healthcare administration field, or related experience
  • Mental health or social work experience
  • Bilingual Spanish

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Oversee the successful implementation of the ACL grant activities in partnership with leadership across POH departments. Responsibilities related to ACL grant include:
    1. Develop timeline, work plan, and tracking system for achieving ACL grant deliverables while ensuring that reporting requirements are met.
    2. Work across POH client-facing departments to coordinate suicide prevention training program designed to reach grant targets and longer-term sustainability goals for POH.
    3. Ensure outcomes listed in the ACL grant are met and provide regular reports and updates to Leadership as needed. Identify and communicate expected adjustments and/or changes in relation to strategy, outcomes, and other targets.
    4. Participate in meetings with ACL funders and other ACL award recipients as needed, including monthly grantee gatherings and annual ACL grantee conferences.
  • Under the supervision and guidance of POH Client Services and Nutrition Services leadership, provide client support in engaging with food is medicine programming. Responsibilities of care navigation as part of the AHA-funded research project include:
    1. Strong knowledge and understanding of community resources to provide general advice to clients and others regarding available services.
    2. Conduct weekly navigation calls for study participants, or support until patient declines.
    3. Assist in making appointments and providing reminders for nutrition assessment and counseling services.
    4. Document adherence and qualitative information gathered during navigation interactions.
    5. Provide educational and support materials, as needed and indicated by the Nutrition Services team, for participants to support use of food and management of health through nutrition.
    6. Coordinate adjustments to text message support for participants with UCSF team
    7. Participate in weekly rounds with the UCSF care navigator to ensure a coordinated approach to participant support and study protocols
    8. Participate in any interviews or other data collection activities for process evaluation of the study intervention.
