The FSC for the Supportive Housing Program will assist clients experiencing homelessness with housing search and placement activities and provide case management services to reach goals. The FSC will have an involvement in local committees such as Housing Support Meeting, HIT, and when needed, the Housing Coalition, and Heading Home Committee.


ESSENTIAL FUNCTIONS:

•Assess client level of need through assessment, determine eligibility for programming, and complete acceptance or denial process with the client.

•Assist client with housing search and placement activities related to their supportive housing program.

•Monitor client progress toward goals. The FSC challenges, assists, councils, advocates for and empowers clients to become self-supporting.

•Develop and write client plans specific to emotional, physical, social and cultural issues in an effort to reduce housing barriers.

•Evaluate client plans at regular intervals in relation to results achieved.

•Work directly with an integrated team of staff from CAP, the County, and other organizations to determine referred program eligibility.

•Respond to client’s defined crisis situations as needed.

•Perform regular visits in the client’s home or in the office/community, as part of on-going case management.

•Maintain accurate and conscientious client files; follow the process detailed by funding source.

•Adhere to requirement for tracking information.

•Calculate rent to be paid by the client according to program guidelines and manage direct service funds for clients.

•Develops housing resources by establishing relationships with landlords and short-term stay residences in order to link clients to them and other local resources.

•Other Duties as assigned.


MINIMUM QUALIFICATIONS

Education, Training and Experience Guidelines

Four-year degree in social work or related field plus one year of associated experience or equivalent combination of education and/or experience. Experience working with individuals experiencing homelessness or families from various social backgrounds.


Knowledge of:

•Applicable housing policies, procedures and regulations covering specific areas of assignment.

•Principles of Trauma Informed Care, Person Centered Theory, and Housing First.

•Knowledge and experience working with mental health and chemical dependency populations.

•Specialized CAP and state required software (HMIS) applications and procedures.

•Local community resources and regional community services programs.

•Principles of record keeping and records management.

•Personal computers utilizing standard and specialized software.


Skill in:

•Analyzing problems and developing effective solutions.

•Principles and practices of conflict resolution, customer service and outreach.

•Working effectively with persons of diverse racial and socioeconomic backgrounds.

•Assessing and prioritizing multiple tasks, projects and demands.

•Perceiving the needs and concerns of others, interacting in a tactful manner, resolving conflicts, mediating disputes, and building personal skills.

•Preparing and presenting public reports and engaging community groups in meaningful dialogue.

•Maintaining composure and working effectively under stressful conditions.

•Collaborating as a team member, with colleagues, staff and community representatives.

•Establishing and maintaining cooperative working relationships with co-workers, clients and community agencies.

•Effective verbal and written communication.


LICENSE AND CERTIFICATION REQUIREMENTS

A valid Minnesota State Driver’s License, proof of insurance and review of Motor Vehicle Record (MVR) report is required.


PHYSICAL DEMANDS AND WORKING ENVIRONMENT

Work is performed in a standard office environment which requires occasionally lifting such articles as file boxes or heavier materials with help from others and/or lifting and carrying light objects frequently. Mobility to drive a motor vehicle in order to attend meetings. A job in this category may require walking or standing to a significant degree or may involve sitting most of the time with long periods of computer work and heavy phone usage.
