As a BHH Care Coordinator, you will enjoy a high level of autonomy and flexibility in managing your workload. Key Responsibilities:


  • Conduct psychosocial assessments to identify client needs and strengths
  • Develop and implement strengths-based treatment plans tailored to each client
  • Coordinate care with other healthcare providers and community resources
  • Promote health and wellness through education and support
  • Advocate for clients' rights and support their families in accessing necessary services
  • Facilitate small group sessions to provide additional support to clients
  • Maintain accurate and timely records of client interactions and progress

Requirements:
  • MHRT-C or MHRT-C Provisional Certification: Current certification or eligibility for MHRT-C/p, with paid training available for qualified candidates.
  • Independent Decision-Making: Strong ability to think independently and make sound decisions.

  • Background Checks: Must pass background checks and not be listed on any state or federal suspension or debarment lists.
