About the Role

We are seeking a highly skilled and experienced Care Coordination Supervisor to join our Behavioral Health team at Multnomah County. As a key member of our team, you will be responsible for overseeing the development and implementation of our care coordination programs, ensuring high-quality services are delivered to our clients.

Key Responsibilities
  • Program Development: Collaborate with program leadership to develop and implement new programs, including the Jail Coordination Team, Substance Use Assessment Team, and Housing Support Specialist.
  • Supervision: Provide administrative and clinical supervision to the Jail Care Coordination Team, Substance Use Assessment Team, and Housing Specialist, ensuring quality of client care and compliance with county and departmental policies.
  • Partnerships: Develop and maintain partnerships with other leadership, community organizations, and state personnel to ensure effective program delivery and quality improvement.
  • Training and Development: Provide ongoing training to staff to enhance clinical expertise and ensure adherence to best practices.
  • Human Resources: Oversee hiring, orientation, and performance evaluations for new and existing staff, including disciplinary action when needed.
Requirements
  • Certification: Must meet the standards of Certified Alcohol and Drug Counselor - II (CADC-II)
  • Education/Experience: Bachelor's degree in a mental health field with major coursework in social science, or a related field, and 3-6 years of experience in behavioral healthcare settings.
  • Preferred Qualifications: Dual certification as Qualified Mental Health Professional (QMHP-C) and CADC-II, as well as experience in program development and leadership.
