Intake Coordinator
Reports to: Clinical Director
Job Summary:
Welcomes new patients to Basecamp Recovery Center and oversees completion of intake documents and tasks. Coordinates with referral partners to facilitate a smooth transition process. Intake Coordinator works alongside operations, medical, clinical, and case management teams to ensure appropriate access to needed treatment resources, promoting patient retention and engagement in treatment, and meets basic needs of patients while also working to eliminate barriers to engagement in recommended treatment services.
Job Duties/Responsibilities:
• Take incoming phone calls for requests for program information
- Perform Eligibility for Services intake interview for all potential clients
- Determine appropriateness for clinical assessment
- Schedule date/time for clinical assessment balanced against open bed availability
- Upon arrival, take incoming clients through entire intake process by scheduling clinical assessment, nursing assessment and benefits review
- Upon arrival, review client handbook, review client insurance information, complete ROI’s
- Upon arrival coordinate housing and arrange transportation
- Respond to phone messages same day
- Develop and enhance relationships with referral sources
- Dedication to being of service to our clients and referral sources
Required Skills/Abilities:
- Proficiency in Google Suite
- Decision making skills
- Time management skills; organized and able to prioritize
- Strong oral/written communication skills
- Interpersonal skills with clients, colleagues, and community resources
- Problem/conflict resolution skills
- Comfortable in a fast-paced environment with multiple clients
- Ability to relate and communicate with a broad socio-economic cross-section of individuals
- Comfortable with building personal relationships and dedicated to helping others
