The Physical Therapist Assistant/Liaison position offers a unique opportunity to blend clinical expertise with community engagement.
Compensation:- PTA Compensation: $34-$40 per visit
- Liaison Compensation: $30/hour | 10 hours weekly | dual role
Location: London, Ohio (surrounding area)
Key Qualifications for the PTA/Liaison Role:
- Must possess a valid Physical Therapist Assistant license
- CPR/BLS Certification required
- Willingness to travel is essential
- Valid Driver's License and auto insurance necessary
- Reliable transportation is a must
- Home health experience is preferred
- Facilitating referrals to the Intake Coordinator
- Engaging with physician offices to obtain therapy orders, skilled nursing, and recent visit documentation
- Overseeing patient progress in collaboration with supervising therapists
- Ensuring adherence to the care plan established by the therapist
- Communicating effectively with patients, their families, and other stakeholders, while updating electronic documentation systems
- Maintaining ongoing caseload lists for all managed accounts, including start of care, reassessment, and discharge dates, and reporting to nursing directors weekly
- Utilizing resources from home health agencies to fulfill obligations for accounts, including assistance from therapists with exercise classes and events
- Managing a weekly caseload of 27-32 visits (full-time)
- Identifying evaluation needs at appropriate intervals and assisting in evaluations under the Physical Therapist's guidance
- Collaborating with the Physical Therapist to establish target lists, short-term and long-term goals, and treatment plans
- Delivering individual treatment as directed by the Physical Therapist, focusing on muscle strength, coordination, mobility, and other therapeutic activities
- Assisting in selecting and adapting physical therapy techniques/media based on client performance
- Fostering positive relationships with coworkers, supervisors, clients, families, and community personnel
