PTA/Liaison

Physical Therapist Assistant/Liaison 

PTA Salary: $34-$40/per visit

Liaison salary: $30/hr | 10 hours weekly| split position

Schedule: Full time | M-F| Flexible hours 

Location: London, Ohio (surrounding area)

Requirements of the PTA/Liaison role:

• Licensed Physical Therapist Assistant

• CPR/BLS Certified

• Must be willing to travel

• Valid Drivers License and Auto insurance Coverage

• Reliable transportation 

• Home health experience preferred 

About the Physical Therapist Assistant/Liaison role: 

• Sending referrals to the Intake Coordinator 

• Communicate with physician offices to request orders for therapy, skilled nursing, and recent visit notes.

• Supervising therapist regarding patient progress

• Responsible for following up with the plan of care set by the therapist

• Communication with patient/POA/patient families, update electronic documentation system

• Keep ongoing caseload lists for all accounts managed, including date of start of care, reassessment, and discharge dates- then send them to directors of nursing weekly

• Leverage resources from home health agencies to help with obligations for our accounts. As well as assistance from the therapist with exercise classes, monthly weights, activities, and events. 

• Maintain a weekly caseload of 27-32 visits (full-time)

• Identifies indications for evaluations at appropriate intervals and assists in the evaluation under the direction of the Physical Therapist.

• Assists the Physical Therapist in identifying a target list, short-term goals, long-term goals, and treatment plan.

• Conducts individual treatment as assigned by the Physical Therapist and outlined in the treatment plan that includes, but is not limited to, areas of muscle strength, coordination, mobility, sensory awareness, sitting and standing, transfer training, balance activities, gait training, range of motion activities and modalities.

• Assists in the selection of physical therapy techniques/media and in the sequence of activities, adapts techniques/media as indicated by client performance and following consultation with the Physical Therapist.

• Maintains positive relationships and rapport with coworkers, supervisors, clients, family members and community personnel, and administration.
