Position Purpose: Families Together of Orange County (FTOC), a rapidly growing Federally Qualified Health Center (FQHC) located in Tustin, CA, is looking for a full-time Patient Outreach & Enrollment Advocate. The advocate will be our boots on the ground to assess, educate, and assist clients in enrolling qualifying individuals and families for multi-programs offered at FTOC. Programs include Medi-Cal, Affordable Care Act (ACA), Cal Fresh, General Assistance/Relief, Family PACT, Every Woman Counts, Sliding Fee Discount Program to name a few. As an advocate, you will represent FTOC in the community by creating and presenting at partner organizations and community events on FTOC outreach and enrollment initiatives. The overall goal is to increase access to quality healthcare for individuals and families residing in Orange County.


Core Duties and responsibilities, include but are not limited to:

  • Identifies individuals who qualify for FTOC services/programs through direct outreach activities and through reports/referrals from community partners, churches, schools, internal data, and others.
  • Assists eligible clients to apply for publically sponsored health care insurance plans using Covered California (Calheers) or BenefitsCal, providing necessary follow-up, tracking and data collection to assure continuous enrollment.
  • Maintains a tracking and follow-up system for Medi-Cal and other social service applications to ensure that all aspects of the process from application through post-enrollment is complete. Ensure the client is able to receive appropriate services.
  • Follows up on all pending applications to ensure enrollment process is complete including successfully enrolled in an IPA/Medical Group and assigned to a Primary Care Provider (PCP).
  • Provides great customer service by assisting patients with any questions or concerns about health coverage and other social service programs.
  • Assists by reaching out to social services or other agencies involved in order to troubleshoot application issues.
  • Secures patient information and maintains patient confidentiality by completing and safeguarding patient records and other patient information
  • Accurately records patient information and documents for application submission.
  • Maintains safe, secure, and healthy work environment by establishing and following standards and procedures complying with legal regulations.
  • Participates in workshops for Medi-Cal application and social programs assistance to the community.
  • Works with local agencies to support further social service needs to patients.
  • Engages with patients on a daily basis to assist with insurance needs such as insurance and Medi-Cal financial counseling.
  • Outreach to raise awareness of the availability and necessity of a Primary Care Provider and health plans.
  • Connects with individuals to inform them about FTOC services, related community resources, accessing needed care that facilitates individual and family health, care coordination, continuity of care, and case management.
  • Provides information in a manner that is culturally and linguistically appropriate for community members. Information given should be complete, fair and impartial.
  • Performs other tasks and duties as assigned.

Education and Training:

  • High School Diploma
  • 1 year in eligibility and verification of healthcare benefits, specifically Medi-Cal or CalOptima Health.
  • CoveredCA Certified Counselor is highly preferred


Qualifications, Skills, and Experience:

  • 1 year of healthcare experience
  • FQHC or community clinic experience is highly preferred
  • BenefitsCal experience is highly preferred
  • Verbal Communication, Organization, Scheduling, Professionalism, Customer Focus, Confidentiality, Bedside Manner, Medical Teamwork
  • Knowledge and experience in working with diverse community members
  • Able to adapt to a dynamic environment
  • Proficient in MS Office and Outlook


Requirements:

  • Bilingual (Spanish)
  • Passionate about helping and empowering others
  • Valid CA Driver License
  • Proof of Satisfactory Driving Record (DMV Print Out)
