Position Title: Clinical Case Manager (RN)
Compensation: $40 per hour, dependent on experience

The Clinical Case Manager will be responsible for evaluating patients with chronic conditions and aiding in the formulation of comprehensive care strategies in partnership with care coordinators, primary healthcare providers, and other essential clinical personnel. This role will involve collaboration with clinic management and will lead the annual competency evaluations for Medical Assistants throughout the organization. Furthermore, the Clinical Case Manager will oversee vaccine administration and adherence to compliance standards.
Key Responsibilities:
- Offer clinical assistance to case managers and medical coordinators, focusing on chronic disease management and medication oversight.
- Serve as a liaison between medical coordination teams and case management to enhance the overall care framework and provide direction on suitable external resources.
- Ensure clinical supervision and monitoring for thorough intake evaluations.
- Engage in the creation of patient-centered care plans tailored for the enrolled individuals.
- Conduct timely medication reconciliation during transitions in care, promoting medication adherence and education.
- Design and implement innovative programs to enhance resource availability for clients, collaborating with community organizations to establish new partnerships and creatively address client needs during Recuperative Care and/or Short-Term Post-Hospitalization.
- Utilize data analytics to assess outcomes from targeted interventions and assist in making necessary adjustments to care plans based on findings.
- Initiate prevention and engagement initiatives.
- Provide health coaching for groups or individual enrollees as required.
- Participate in quality improvement initiatives.
- Current Registered Nurse (RN) license in California.
- Experience working with at-risk populations, including individuals with trauma histories, those facing socioeconomic challenges, homelessness, or substance use issues.
- Excellent interpersonal abilities and clinical problem-solving skills.
- Comfortable operating in an independent clinical environment and engaging in outreach nursing.
- Proficient in data utilization for performance tracking and measurement.
- Basic computer proficiency, including email communication and data summarization in spreadsheets.
- Valid California Driver's License and eligibility for company vehicle insurance.
- Bachelor of Science in Nursing.
- Experience in Care Management.
- Bilingual proficiency in English and Spanish.
- Familiarity with Microsoft Office Suite (Outlook, Word, Excel, Teams).
