Role Objective: Families Together of Orange County (FTOC), a dynamic Federally Qualified Health Center (FQHC) situated in Tustin, CA, is seeking a dedicated full-time Healthcare Enrollment Advocate. This role is essential in engaging with the community to evaluate, inform, and support clients in the enrollment process for various programs offered by FTOC. These programs encompass Medi-Cal, Affordable Care Act (ACA), Cal Fresh, General Assistance/Relief, Family PACT, Every Woman Counts, and the Sliding Fee Discount Program, among others. The advocate will actively represent FTOC within the community by delivering presentations at partner organizations and community events focused on outreach and enrollment initiatives. The primary aim is to enhance access to quality healthcare for individuals and families residing in Orange County.
Key Responsibilities:
- Identify individuals eligible for FTOC services/programs through proactive outreach and collaboration with community partners, educational institutions, and other referral sources.
- Assist qualified clients in applying for publicly funded health insurance plans via Covered California (Calheers) or BenefitsCal, ensuring thorough follow-up, tracking, and data collection to maintain continuous enrollment.
- Implement a systematic tracking and follow-up process for Medi-Cal and other social service applications to guarantee that all stages from application to post-enrollment are completed effectively.
- Conduct follow-ups on pending applications to confirm the completion of the enrollment process, including successful enrollment in an IPA/Medical Group and assignment to a Primary Care Provider (PCP).
- Deliver exceptional customer service by addressing patient inquiries regarding health coverage and additional social service programs.
- Collaborate with social services or other agencies to resolve application-related issues.
- Safeguard patient information and maintain confidentiality by managing patient records and other sensitive information appropriately.
- Accurately document patient information and submissions for applications.
- Foster a safe and healthy work environment by adhering to established standards and legal regulations.
- Participate in workshops aimed at assisting the community with Medi-Cal applications and social programs.
- Engage with local agencies to support patients' broader social service needs.
- Interact with patients daily to assist with insurance inquiries, including financial counseling related to Medi-Cal.
- Conduct outreach to promote awareness of the importance of having a Primary Care Provider and available health plans.
- Connect with individuals to inform them about FTOC services, community resources, and facilitate access to necessary care, ensuring continuity of care and effective case management.
- Provide information in a culturally and linguistically appropriate manner, ensuring it is comprehensive, fair, and impartial.
- Perform additional tasks and responsibilities as assigned.
Education and Qualifications:
- High School Diploma
- Minimum of 1 year experience in eligibility and verification of healthcare benefits, particularly Medi-Cal or CalOptima Health.
- CoveredCA Certified Counselor certification is highly preferred.
Skills and Experience:
- At least 1 year of experience in the healthcare sector.
- Experience in a Federally Qualified Health Center or community clinic is highly preferred.
- Familiarity with BenefitsCal is highly preferred.
- Strong verbal communication, organizational skills, scheduling abilities, professionalism, customer focus, confidentiality, bedside manner, and teamwork.
- Experience working with diverse community members.
- Able to adapt to a fast-paced environment.
- Proficient in Microsoft Office and Outlook.
Requirements:
- Bilingual (Spanish)
- Genuine passion for helping and empowering others.
- Valid California Driver License.
- Proof of satisfactory driving record.
