Extended Foster Care Program Overview

Ohana is launching its Extended Foster Care (EFC) program in direct partnership with the Arizona Department of Child Safety that will serve young adults in foster care starting at 17.5 years old. Ohana’s EFC program will work directly with young adults to build and maintain a relationship with them that seeks to encourage them to voluntarily enter and participate in the state of Arizona’s Extended Foster Care program. As part of this program, Ohana Success Coaches will walk alongside these young adults until their 21st birthday, helping to prepare them for a successful transition into adulthood and beyond.

The Extended Foster Care Program is an exciting new support service that Ohana is proud to provide. In order to fulfill our mission, Ohana is currently seeking full-time Success Coaches to join our team.


Success Coach Job Description

The Success Coach will be responsible for developing and maintaining a supportive relationship with the young adults that demonstrates respect through listening, asking motivational questions, approaching them with curiosity (not judgment), encouraging them, helping them set achievable goals, and respectfully reinforcing young adults and their personal responsibility in the program. The Success Coach will also be responsible for promoting permanent connections, including connecting to and maintaining relationships with mentors and other significant adults in their life. The Success Coach supports the development of an educational foundation and skill set that will enable young people to gain and maintain employment that meets their financial needs. The Success Coach also ensures that young adults reside in safe, stable, and secure housing. Success Coaches will collaborate with both Ohana and community based services and resources to assist the young adults in reaching their goals in the previously listed areas. In addition, the Success Coach links young adults to appropriate services to address physical and behavioral health needs through seeking community partnerships that help build skills for the development of personal agency. The Success Coach also ensures that young adults have the cognitive and social-emotional competencies essential to the ability to thrive. This position will report directly to an EFC Program Manager.


Primary Functions

  • Carry a caseload of up to 20 young persons.

  • Exhibit the belief that all young people have the capacity to be successful in their lives.

  • Complete Clinical Supervision with a supervisor for each case at least once a month and when key decision-making points are needed for each young adult.

  • Conduct weekly check-ins with direct supervisor to discuss any needs identified.

  • Receive and work all cases located throughout Arizona assigned by direct supervisor.

  • Engage young adults in a coach-like way that supports them during periods of crisis, assists with establishing goals, securing safe and stable housing, and makes referrals on their behalf for appropriate services or community support.

  • Work directly with young adults prior to their 18th birthday to sign a voluntary agreement.

  • Complete minimum of thirty-five (35) in-person visits at the young adults’ residence (at minimum of 30 minutes in length) each month of service to provide support services and determine additional meetings as determined by the young adult and their specific needs.

  • Complete a monthly child contact note in Guardian within two (2) business days of any contact with the young adult.

  • Organize and lead the young adults’ Transition Plan staffing with appropriate team members.

  • Support and participate in the EFC Reviews.

  • Complete at a minimum one (1) monthly collateral contact with professionals.

  • Access DCS Guardian system is required to register and take all necessary computer-based trainings prior to receiving access to ADCS Child welfare case management System.

  • Responsible for completing all initial outreach, Monthly Engagement Meeting, monthly engagement contact documentations that are submitted to DCS for billing.

  • Transport young adults as needed, which includes transporting the child(ren) of the young adult if needed to accomplish a task or goal with and prevent barriers to service.

  • Respond to non-emergency outreach made by the young adult and be part of a rotating on-call emergency response.


Minimum Requirements

  • Flexible, solutions-oriented attitude and be comfortable trying new approaches.

  • Knowledge of trauma-informed care and healing-centered approaches.

  • Understand the community resources and providers to ensure successful service referrals.

  • Shall possess a bachelor’s or associate degree in appropriate discipline or a success coach may possess skills that the coach acquired through alternative routes such as relevant job training, community college attendance, military service, or an apprenticeship.

  • Shall be at least 26 years of age.

  • Complete Youth Thrive Training

  • Current CPR certification

  • A valid driver's license that has not been suspended or revoked due to a moving violation within the most recent thirty-nine (39) months and valid car insurance.

  • Cleared Central Registry Background Check and a Level One fingerprint clearance card.

  • Complete annual ongoing training to meet contractual requirements.
